Our Office hours:

We adhere to the scheduled appointments (no walk-ins) from 8:00am-5:00pm. To honor our patients with existing appointments, walk-in patients will be offered the next available appointment time and may be charged a convenience walk-in fee if seen immediately. We do not do Walk-ins during the office hours, this will be discouraged in our practice to be respectful of those with appointments. We will need to handle this on a case-by-case basis if it occurs in appointment time only hours. If you are going to be late for an appointment or need to cancel, please call our main office number 678-381-2630 prior to the appointment.

To reach us during office hours please call 678-381-2630. Calls are answered by the staff, who will attempt to answer your question and/or give you an appointment. Questions which cannot be answered by the staff will be forwarded to the patient’s provider. Urgent calls will be handled immediately. Non urgent calls will be returned by the provider as soon as possible.

We welcome new patients transferring into our practice and want to help make this transition as seamless as possible. All new patients, regardless of their age, must be accompanied by their parent or legal guardian for the initial visit. If possible, please bring a copy of your child’s previous medical records for our providers to review. Each NEW PATIENT appointment is scheduled for 30-45 minutes for our providers to adequately review your child’s medical history and complete a physical exam. Please plan to arrive 30 minutes prior to your appointment if you have not filled out all the new patient paperwork, 20 minutes early if you have filled it out. We encourage you to schedule your NEW PATIENT appointment as soon as possible in order to be considered an established patient. However, for your child’s safety, we cannot offer medical advice over the phone until your child has been seen in the office.

Our After hours and Weekends:

If it is an emergency or of a life threatening nature or illness, please call 911. If you have medical needs after hours, there are multiple locations available for pediatric care.
We believe in children receiving pediatric care and encourage you to use the Scottish Rite Children’s Hospital, Egleston Children’s Hospital for any of your emergency or after hours needs.

There is also urgent care office operated by the Children’s Hospital, Satellite Boulevard is the closest location to our office. 2660 Satellite Blvd NW, Duluth, GA 30096(404) 785-8590.

Insurance:

Our office will be accepting: Aetna, Coventry, Blue Cross Blue Shield/ BCBS (PPO, HMO, POS), Cigna, Human, United Healthcare, Georgia Medicaid, Peachstate CMO, and many more. Please contact us if you have a specific need regarding insurance.

Prescription Refills:

Prescriptions can be refilled via requesting on the patient portal, we encourage you to use this method as it is secure.

Certain medications cannot be phoned in to a pharmacy, such as ADD medication and they require the patient to have an original prescription. We ask that you give us at least a minimum 24 hour notice for any refill requests on ADD medication. When the prescriptions are ready, they will be available for pick up at the front desk. Some children may be required to come in for follow up, depending on the situation, before the refill on the ADD medication will be given.

It is important that the child comes to the office to receive an examination and an accurate diagnosis, therefore, we do not fill new prescriptions or make diagnoses over the phone.

New Patient Forms:

Prior to your first visit, after obtaining baseline information, time allowing, you will be directed to the patient portal to fill out new patient information. Or you may print and fill out a copy of our new patient form and bring it with you to the office. On your first visit, you will sign a records request for transfer release form for your previous provider and your child’s records can be forwarded to our office. This request can only be done by the parent/guardian. You can do this prior to your first appointment if you want.

Forms Request Policy

Due to the extremely large volume of requests for medical records copies, first copy is free and any additional copies will require a fee, according to the Georgia code.

For medical forms requests, please refer to the financial information for medical records requests link by clicking here and Georgia law.

For sports physical forms, hearing and vision forms, camp forms, immunization records, at the current time, if you request these at your child’s appointment time, there is no charge. If you request these after the appointment, it may need to wait until the end of the day for completion. If you are requesting at a non-appointment time, please allow 48-72 hours for completion. Please remember to plan ahead for registrations, etc. These forms are completed on a ‘first in/first out’ basis. We make every effort to complete these forms as quickly as possible. Due to the heavy volume of requests during back-to-school months, processing times may be longer. Please, please, please plan ahead!!

Parents are REQUIRED to complete all information on the forms that they have knowledge of (i.e. NAME, date of birth, allergies to medications/foods, any demographics, health history, etc), and the parent is REQUIRED to sign and date their portion of the form. IF these forms are received incomplete forms may delay the processing of your forms.

Parents will be required to provide self-addressed, stamped envelopes or pick up the forms at the office. Due to HIPPA Privacy regulations, we will no longer return completed forms via fax.
